📍Companion/Sitter Services

Our Companion and Sitter Services provide supervision, companionship, and non-medical support to individuals who may benefit from having a dependable caregiver present.

Services may include:

Companionship and social interaction
Supervision and safety monitoring
Assistance with routine daily activities
Support with maintaining independence
Engagement in meaningful activities
Support for individuals who should not remain alone for extended periods
Respite and support for family caregivers

📍Skilled Nursing Services

We provide nursing services delivered by qualified licensed professionals based on the individual’s assessed needs and authorized plan of care.

Registered Nurse (RN) Services

Nursing assessments
Health monitoring
Nursing oversight and coordination
Care-plan implementation and monitoring
Education and support for individuals and caregivers
Other authorized skilled nursing services

Licensed Practical Nurse (LPN) Services

Authorized nursing care
Monitoring of health status
Implementation of nursing interventions

Services provided under appropriate nursing supervision and according to the individual’s plan of care

📍Community Living Supports (CLS)

Community Living Supports help individuals with intellectual and developmental disabilities develop, maintain, and strengthen the skills needed to live successfully in their home and community.

Support may focus on:

Daily living skills
Personal independence
Community participation
Household routines
Personal safety
Social and interpersonal skills
Self-direction and choice
Increased independence in everyday life

📍Community Access Individual

Community Access Individual services provide individualized support that helps participants become more involved, independent, and connected within their communities.

Services may include support with:

Community participation
Developing social skills
Building community relationships
Recreational and leisure activities
Developing independent living skills
Accessing community resources
Increasing confidence and independence outside the home
Pursuing individually identified goals and interests

📍In-Home Respite Services

In-Home Respite provides temporary support for individuals while giving their regular family member or unpaid caregiver time away from caregiving responsibilities.

Respite services help:

Maintain the individual’s health and safety
Provide supervision and assistance in the home
Support established routines
Give family caregivers temporary relief
Reduce caregiver strain
