RN Supervisor/Coordinator

We Care Home  Care has an opening for a Registered Nurse for home health visits and case management. 

 

Job Description: Oversee the operation of home care services. The services are geared to the goal of a maximum quality of life for each client.

 

Qualifications:

  1. A current license to practice as a registered nurse in Georgia. 

  2. Basic first aid and CPR

  3. Completion of appropriate training for supervisors for each specific type of service and service plan development

  4. Two (2) years’ experience in home health or a related field. 

  5. Satisfactory criminal background check

  6. Experience working with the elderly, physically and mentally disabled, and veterans are preferred. 

 

Duties & Responsibilities:

1. Only RN’s can be responsible for supervising any services provided for medically frail clients and defines those types of clients appropriately.

2. To agree to supervise in a manner that is consistent with PHCP/DCH regulations and the Policies set forth by We Care Home Care

3. To annually assess the performance of the supervisees and staff, by direct observation or demonstration of the tasks they are assigned to perform.

4. Requires and documents that the supervisor participates in the development and review of each client’s service plan.

 

Companion/Sitter

 

Qualifications:

Must be able to read, write, and follow instructions and completion of training or pass competency assessment, as appropriate for understanding the needs of the population served.  Must participate in the orientation and training program of the Agency.  Must be able to pass a criminal background check and a physical and TB test. CPR/First Aid Certified.

 

Education and Experience

 

a. Experience or related experience

b. Must be 21 years of age

c. Current valid driver’s license

 

d. Current CPR/First Aid Certificate

e. Current Physical Examination

f. Current Negative TB Screen

g. Satisfactory Fingerprint and Criminal Record Check

 

Duties & Responsibilities:

  • Basic meal preparation

  • Provision of transportation services (running errands)

  • Housekeeping

  • Home safety

  • Handling emergencies in the home

  • Infection control policies and procedures

  • Observe and report changes in client condition

 

Certified Nurse Aide

 

Position Qualifications:

 

  1. 18 years of age or older.

  2. Be able to read and write; follow verbal and written instructions and complete written reports of care given.

  3. Must be able to communicate with the client.

  4. Must be competent to perform the tasks assigned.

  5. Evidence of sympathetic attitude toward the care of the sick.

  6. 40-hour training program completion

  7. Basic first aid and CPR

  8. Completion of nurse’s aide training

  9. Satisfactory criminal background check

 

Duties & Responsibilities for CNA:

 

  • Ensures quality and safe delivery of Home care services.

  • Participates in the development and implementation of client Service Plan per Home Care Agency policy and procedures.

  • Information regarding the client Service Plan is submitted to the Home Care Nursing Supervisor in a timely manner.

  • Client clinical record documentation reflects the delivery of quality and safe Home Care Services.

  • Report on the personal needs of the client

  • Report on any change in the client condition and document and record

  •  Implement current Personal Care Services.
